Create a FACTS Account
FrenchAm has partnered with FACTS Management to manage all tuition and incidental payments.
FACTS is the largest provider of tuition payment plans in the industry, giving students and families from educational institutions at all levels a better way to manage education costs over time.
With a FACTS, you can choose a convenient payment option to suit your needs. Instead of one lump tuition payment, smaller payments can be made over time.
FACTS is the only method of payment accepted by FrenchAm.
Required Deposit
As you have seen in the application you just completed, a $1,500 per student, is due at time of enrollment.
In the process of creating your FACTS account, you will be prompted to process your deposit. You can choose to pay with a credit acrd of check draft. Just follow the steps to select your preferred method of payment.
Complete and Submit Emergency, Information and Immunization Record Card
The State of Arizona requires all parents to complete an Emergency, Information and Immunization Record Card (We call it “911 Form”).
You can download the form below, then fill and print. It then must be signed and uploaded back to this web site. You can also, if you choose so, mail us the form, or drop it at the front desk.
The 911 Form must be returned to our Admissions Office before the first day of school. No student may join the Preschool, without this form.
Submit Latest Immunization Record
The State of Arizona requires all pupils enrolled in school to have state required immunizations.
If you don’t already have one, get a copy of your child’s latest immunization record from your family practitioner. You can also ask your doctor to fax the form to (480) 631-7885.
If you have the immunization record handy, just upload it using the form below.
Immunization Record or Exemption Form must be returned to our Admissions Office before the first day of school. No student may join the Preschool, without these forms.
Immunization Exemption
Arizona Department of Health Services (ADHS) strongly supports immunization as one of the easiest and most effective tools in preventing diseases that can cause serious illness and even death. ADHS also respects the rights of parents who are raising their child in a religion whose teachings are in opposition to immunization to make the decision not to vaccinate their child.
Upload Birth Certificate
Unless you have already done it when you completed the online application form, please upload your child’s birth certificate here.
Copy of Birth Certificate must be returned to our Admissions Office before the first day of school. No student may join the Preschool, without this form.
School Communication with Parents
FrenchAm uses ParentSquare for school communication, primarily with email, text and app notifications. ParentSquare automatically generates an account for each parent, using their preferred email address and phone number.
Once you application is complete, you will receive an invitation to join ParentSquare. Make sure to do it, as ParentSquare is the only method used by FrenchAm to communicate with parents.
This will allow you to receive school and teachers’ newsletter, communication, and much more.
Here’s what you can do with ParentSquare:
- Receive messages from the school via email, text or app notification
- Choose to receive information as it comes or all at once with a daily digest at 6pm
- Communicate in your preferred language
- Comment on school postings to engage with your school community
- Direct message teachers, staff and other parents
- Participate in group messages
- Sign up for parent-teacher conferences
- Send payments, sign forms & permission slips, receive report cards, sign up to volunteer and more all from your desktop or mobile device
